Chronic Pain Support Nurse

Job Summary

The Chronic Pain Support Nurse collaborates with a multidisciplinary team to improve safety for patients with chronic, non-cancer pain who are being treated with long-team opioids. The Chronic Pain Support Nurse is an integral member of the Patient Care Team which includes physicians, nurse practitioners, nurses, behavioral health professionals and administrated staff. The role of the Chronic Pain Support Nurse is to work, at the highest level of function allowable, as part of the Patient Care Team to provide for optimal outcomes and enhance the care provided to patients.


The Chronic Pain Support Nurse must demonstrate organizational, communication, spread sheet and database skills. Must be comfortable with use of technology, including electronic health record and database management. Working as an effective member of a team as well as working independently is required. Graduation from an accredited School of Nursing and unencumbered Massachusetts RN license is required. 3+ years of nursing experience required. Psychiatric, pain clinic or addiction medicine experience preferred. Bachelors of nursing degree is preferred.

Representative Job Duties

Assists with providing patient care: records vital signs, discusses and documents chief health concerns
• Coordinates patient prescriptions.
• Acts as a liaison between nursing agencies, VNA and other health care agencies.
• Responds to telephone and EHR requests.
• Acts as a liaison to facilitate coordination of patient referrals and forms to other specialties.
• Performs substance abuse screening utilizing an electronic screening tool.
• Arranges follow up visits with patients to discuss issues related to pain management, controlled substance agreement, aberrancies in behavior or urine drug screens, and for care coordination of external resources.
• Deals discreetly with sensitive information, maintaining confidentiality at all times
• Track the need for and call patients for unscheduled pill counts and urine drug screenings
• Provide meaningful feedback to prescribers regarding evidence-based best practices
• Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place
• Work collaboratively and effectively within a team
• Establish positive, supportive relationships with participants and provide feedback
• Help clients in utilizing resources
• Facilitate communication and coordinate services between providers
• Motivate patients to be active, engaged participants in their health
• Effectively work with people (staff, patients, providers, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions
• Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff
• Continuously expand knowledge and understanding of community resources, services and programs provided; human relations and the procedures used in dealing with the public as part of a service or program; volunteer resources and the practices associated with using volunteers, operations, functions, policies and procedures associated with the department or program area, procedures and resources available to handle new, unusual or different situations
• Identify and apply appropriate role definition and skilled boundaries
• Other duties as assigned
