As an Integrated Behavioral Health Clinician (BHC), you will engage patients in comprehensive mental health assessments, co-create treatment plans and goals, provide supportive counseling, crisis intervention, and brief therapy. You will serve a diverse population with varying clinical needs, collaborating with both patients and interdisciplinary team members to promote optimal health outcomes. Additionally, you will act as a liaison between medical staff and patients/families, offering interventions to reduce stress, enhance coping strategies, and facilitate recovery.

Qualifications

  • Licensed as an LCSW/LICSW or LMHC in the state of Massachusetts.
  • Experience in integrated behavioral health settings preferred.
  • Strong assessment, counseling, and crisis intervention skills.
  • Ability to collaborate effectively with interdisciplinary teams.
  • Commitment to addressing mental health disparities and promoting equitable care.

Representative Job Duties

Clinical Care Coordination:

  • Provide direct care to individuals, families, and groups, offering advocacy, referrals, and resources to address social and environmental challenges.
  • Communicate psychosocial insights to physicians and care teams during rounds and document appropriately.
  • Serve as a liaison with external agencies, legal authorities, and referral sources.
  • Maintain a thorough understanding of resources and services to ensure seamless patient care transitions pre- and post-discharge.
  • Stay current on state (Medicaid) and federal (Medicare) requirements and resources impacting patient care continuity.

Crisis Intervention:

  • Support the care team during crises, including medical emergencies, child/adult protection issues, substance use concerns, suicide attempts, trauma cases, and instances of abuse or neglect.

Psychosocial Screening & Assessment:

  • Conduct screenings to identify psychosocial needs and opportunities for behavioral health interventions.
  • Complete thorough psychosocial assessments, document in electronic health records, and communicate findings with the care team.

Routine Interventions & Counseling:

  • Provide direct counseling (in-person and via telemedicine) to individuals, families, and groups, offering psychoeducation and emotional support.
  • Facilitate patient advocacy, coordinate continuum of care, assist with end-of-life counseling, and provide support for new diagnoses or adjustments to illness.
  • Conduct family meetings, offer grief and bereavement counseling, and refer patients to appropriate community resources.
  • Collaborate with patients and families to create tailored intervention plans and educate them about available resources and support systems.

Continued Education & Professional Development:

  • Participate in staff development programs, in-service training, and educational initiatives.
  • Contribute to research activities, departmental projects, and quality improvement studies.
  • Offer peer supervision to newly hired clinicians and provide training to social work students, medical residents, and other healthcare professionals.
  • Stay updated on best practices, emerging research, and evolving treatment modalities.
