SCREENINGS AND IMMUNIZATIONS FOR PREVENTATIVE CARE
This chart shows South Boston Community Health Center's general recommendations for well-care visits (preventative care) for adults. Your own schedule may differ based on your health and personal risk factors. Talk to your provider about what is best for you. Recommended tests and immunizations can be done during your well-care visit or at appointment in between those visits.

*Women over 25 who are at high risk for having an STD also should be screened. Ask your provider what screening schedule is best for you. If you are pregnant, talk to your doctor about the need for chlamydia testing.
**Once in a lifetime, everyone should get a tetanus-diphtheria (Td) booster that contains pertussis (Tdap).
