Health Insurance Services
Assisting you in accessing the care you need.
The SBCHC Financial Counseling department helps patients to:
- Submit a Medicaid (MassHealth) application
- Submit a Medicaid (MassHealth) Renewal form
- Elect or change an insurance-assigned PCP
- Select a MassHealth health plan
- Submit supporting documentation to MassHealth, such as proof of identity, income, or residency
- Address general Medicare questions
If you need assistance with any of these tasks, book a Financial Counseling appointment by calling (617) 269-7500.
Once approved for Medicaid (also referred to as "MassHealth" in the state of Massachusetts), you may be eligible to select a MassHealth health plan. It's important to choose a plan that's best for you and your family. That depends on where you live and where you receive primary care and specialty services.
WellSense Community Alliance (previously called BMC HealthNet Community Alliance) is the MassHealth plan that is accepted at South Boston Community Health Center.
Also, most plans required you to have an assigned Primary Care Provider (PCP). In order for your SBCHC visit to be covered by insurance, you must elect a South Boston Community Health Center provider to be your PCP prior to being seen. To elect or change an insurance-assigned PCP:
- Log in to your MA portal at https://www.mass.gov/masshealth-ma-login-accounts,
- Contact MassHealth Customer Service at 1-800-841-2900; or
- Book an appointment with our Financial Counseling department by calling (617)269-7500 so that a staff member can assist you with contacting your insurer.
For a list of SBCHC primary care providers, visit our Providers page.
At South Boston Community Health Center, we help people apply for state-sponsored health coverage. The process starts with a single, state-wide application that will screen a person for eligibility for MassHealth coverage, ConnectorCare Plans, and Qualified Health Plans based on household size and income. You can apply online at MAHealthConnector.org.
Our Certified Application Counselors can help you with your application if you need it. Book an appointment with Financial Counseling by calling (617) 269-7500. With this allotted time, our staff will guide you through the application process and prepare any other necessary documents needed for eligibility determination.
Coming prepared with your Social Security Card (if applicable), proof of identity, citizenship and residency will expedite your application process.
Acceptable Forms of Documentation
After submitting your application to MassHealth, uninsured patients can access SBCHC services while their application is pending.
Those who do not wish to apply for health insurance may pay for their office visit out-of-pocket at the time of registration. The patient will be required to sign a Financial Responsibility Form stating that they will be responsible for any additional procedures that may occur in the visit.
At South Boston Community Health Center we truly believe in eliminating barriers to care. If for some reason you are not eligible for any of the coverage plans listed on our Accepted Insurance Carriers page, please schedule an appointment with Financial Counseling to discuss other options, including discounts on SBCHC-provided services.
Patients with private or employee-sponsored insurance plans wishing to apply for a secondary coverage through the state can also book an appointment with Financial Counseling. Our Certified Application Counselors can walk you through an online application for additional coverage that could help with your current copayments.
