MassHealth Members

If you haven’t checked your eligibility status recently, come see one of our health insurance advisors. They can quickly reassure that your coverage is active, or assist in any required paperwork that may be hindering your case. When you access Health Insurance Services first, any questions that may have occurred during check-in will have been answered.

Also, you must have South Boston Community Health Center listed as your Primary Care Provider (PCP) with your insurance carrier prior to being seen. To change your PCP, please call MassHealth Customer Service at 1-800-841-2900 and follow the prompts 2, 5, 4, 1, or visit one of our advisors to receive assistance in making the phone call.

Please Note: We encourage patients to arrive 30 to 60 minutes before their scheduled appointment to address any health insurance questions (within our scheduled office hours). Our advisors serve patients on a first-come, first-serve basis.

 

Uninsured

Requesting health care coverage in Massachusetts is easier than you might think, and we’re here to help! The process starts with a single, state-wide application that will screen a person for eligibility for MassHealth coverage, ConnectorCare Plans and Qualified Health Plans based on household size and income. You can apply online at MAHealthConnector.org or visit our health insurance advisors. They have been trained to guide you through the application process and prepare any other necessary documents needed for eligibility determination.

Uninsured patients may schedule appointments at the Health Center so long as they seek Health Insurance Services 60 minutes prior to the visit. With this allotted time, our advisors will help you fill out an application and answer any questions you may have. After completion, a Financial Responsibility Form must be signed, stating that the patient will be responsible for the cost of the visit, should he or she be determined ineligible for coverage by MassHealth and the Health Connector.

Coming prepared with your Social Security Card (if applicable), along with proof of identity, citizenship and residency will expedite your application process.

Acceptable Forms of Documentation

Identity
  • U.S. Passport (even if it is expired)
    will satisfied both Identity and Citizenship
  • Driver’s License
  • Legal Permanent Resident card
  • Certificate of U.S. Citizenship
  • Certificate of Naturalization
  • Military ID
  • School ID
  • A government issued ID containing the individual’s photo or other identifying information
  • Military dependent’s ID
  • U.S. Coast Guard Merchant Mariner card

Residency
  • Utility bill or work order dated within the past 60 days
  • Copy of deed and record of most recent mortgage payment
  • Statement from a homeless shelter

  • Section 8 agreement
  • Homeowner's insurance agreement
  • Copy of lease and record of most recent rent payment
  • Notarized affidavit supporting residency

 

Those who do not wish to apply for assistance in paying for health insurance may pay for their office visit at the time of registration. The patient will be required to sign a Financial Responsibility Form stating that he or she will be responsible for any additional procedures that may occur in the visit.

At South Boston Community Health Center we truly believe in eliminating barriers to care. If for some reason you are not eligible for any of the coverage plans listed on our Accepted Insurance Carriers page, please contact Health Insurance Services to discuss the option of a sliding fee scale.

Insured

Currently insured patients wishing to apply for secondary insurance through MassHealth should also visit our health insurance services department. Our advisors can walk you through a quick online application for additional coverage that could help with your current copayments.

Any concerns about your HMO or PPO plan can be addressed by our Billing department. Our staff specialize in particular insurance plans in order to answer your questions in a personal and efficient manner.
