About Internal Medicine at South Boston Community Health Center
The Internal Medicine Department (also known as Adult Medicine) at South Boston Community Health Center provides comprehensive primary care to adults ages 18 and older. This includes routine physicals, same day visits for urgent problems, preoperative physicals, and routine follow-up treatment of chronic problems such as asthma, diabetes, high blood pressure, obesity and mood disorders.
As part of our routine medical exams, we provide:
- Counseling on healthy lifestyles
- Cancer screenings including breast, colon, cervical and lung cancer
- Screening for diabetes and high cholesterol
- STD screening
- Blood pressure screening
- Family and social history review
- Counseling on family planning and contraception
- Routine immunizations
We also help coordinate specialty referrals, social service supports, and home care when appropriate.
We strive to build therapeutic relationships with patients and their Primary Care Provider (PCP). Your medical team is comprised of providers (physicians, nurse practitioners, physician assistants and internal medicine residents) with the support of our nursing and medical assistant staff. Our team based approach allows us to coordinate care between providers and ensure continuity with your PCP.
