About The Adult Medicine Program at South Boston Community Health Center
The Adult Medicine Program at South Boston Community Health Center provides comprehensive primary care to adults aged 18 and older. This includes routine physicals, same day visits for urgent problems, preoperative physicals, and routine follow-up treatment of chronic problems such as asthma, diabetes, high blood pressure and chronic heart and lung diseases.
As part of our routine medical exams, we provide:
- screening for cancer
- diabetes
- hypertension and other chronic diseases
- cholesterol screening
- counseling on healthy lifestyles
- immunizations.
We also help coordinate specialty referrals and home care when appropriate.
Each care team in Adult Medicine is led by a Medical Doctor along with Nurse Practitioners, Registered Nurses, Care Managers, and Medical Assistants working together to ensure seamless continuity. One of the major reasons we have care teams at the Health Center is so you will always meet with a provider who knows you. For example, if your primary care provider (PCP) is ever not available for an appointment with you, one of their team members, familiar with your health and treatment goals, will see you instead.
