Position Summary

The Nurse Practitioner Fellow is a position within our APRN Transition-to-Practice Fellowship, which provides newly graduated nurse practitioners with postgraduate training within the health center. NPs in this position will both partner with primary care teams to assist PCPs in managing their panels of patients, as well as work toward building and managing a panel of their own patients.

Job tasks include phone and MyChart message triage, prescription refills, care coordination, and office visits. Through their participation in the Fellowship, NPs in this role will grow their skills in triage, differential diagnoses, assessment and plan, and management of acute and chronic medical conditions.

NP Fellows will evolve their NP practice throughout the Fellowship, working toward independent management of increasingly complex patients in office visits. Participation in the Fellowship includes didactic instruction and dedicated precepting by experienced PCPs (MDs and NPs).

Fellows in this position who show excellence in their skills could transition to a non-Fellow NP position at the end of the Fellowship (typically 18-36 months).

Qualifications

  • Master of Science in Nursing or Doctor of Nursing Practice degree from an accredited school of graduate nursing.
  • Active license for Registered Nurse and Nurse Practitioner in the Commonwealth of Massachusetts.
  • Newly graduated NPs are welcome and encouraged to apply!

Preferred Qualifications

  • Experience in a primary care, outpatient, or community health setting
  • Experience working with diverse or underserved populations
  • Bilingual skills preferred but not required

Physical Requirements

  • Ability to bend, lift, pull, and carry up to 70 pounds
  • Ability to sit, stand, and walk for extended periods

Representative Job Duties

  • Consistently and effectively provides timely, safe, and appropriate patient/family care.
  • Involves the patient, significant others and appropriate health care providers in collection of data when appropriate.
  • Consistently uses clear, concise, and effective written/oral communication and complies with departmental policies when sharing relevant patient care data.
  • Demonstrates critical thinking skills, derives and documents appropriate diagnoses from patient and family assessment.
  • Identifies expected outcomes individualized to patients and provides follow-up care as needed, documents these clinical and educational outcomes consistent with the patient’s diagnoses and capabilities and educates patients and families on treatment of illness, health maintenance and prevention.
  • Demonstrates the knowledge and skills necessary to provide age-specific care and consults appropriate resources when serving patients outside the Center’s regular patient population.
  • Effective and appropriate delegates patient care to coworkers or outside agencies as needed.
  • Provides appropriate follow-up care as needed.
  • Practices in compliance with established practice standards, policies, procedures, protocols, and regulations. Completes all mandatory requirements.
  • Provides education to patients and family, focusing on self-care appropriate to their age and identified learning considerations and needs.
  • Demonstrates an ability to remain calm, perform effectively and professionally during peak periods of activity and emergency situations.
  • Acts as a resource for Registered Nurses, Medical Assistants, and support staff as needed.
  • Assumes other duties as assigned.

Competitive benefits package. $112,000-$130,000 per year
