Well-Child Schedules

SCREENING AND IMMUNIZATIONS FOR PREVENTATIVE CARE

This chart shows South Boston Community Health Center's general recommendations for well-care visits (preventative care) for children. Your own schedule may differ based on your health and personal risk factors. Talk to your provider about what is best for you. Recommended tests and immunizations can be done during your well-care visit or at appointment in between those visits.

Birth Newborn blood screen

  • Hep B (hepatitis B)
3-5 days Well-Child Visit
7-14 days Well-Child Visit
1 month Well-Child Visit

  • Hep B
2 months Well-Child Visit

  • [DTaP (diphtheria, tetanus, acellular pertussis), Hib
    (Haemophilus influenzae type b), and IPV (polio)]**
  • PCV (pneumococcal disease)
  • Rotavirus (given by mouth)
4 months Well-Child Visit

  • [DTaP, Hib, IPV]
  • PCV
  • Rotavirus (given by mouth)
6 months Well-Child Visit

  • [DTaP, Hib (if needed), IPV]
  • PCV
  • Rotavirus (given by mouth)
9 months Well-Child Visit

  • Hep B
12 months Well-Child Visit

  • MMR (measles, mumps, and rubella), but not before 1st birthday
  • Hep A (hepatitis A), but not before 1st birthday
  • Varicella (chickenpox), but not before 1st birthday
  • Tests: CBC, Lead
15 months Well-Child Visit

  • DTaP
  • Hib
  • PCV
18 months Well-Child Visit

  • Hep A
2 years Well-Child Visit

  • Tests: CBC, Lead
2.5 years Well-Child Visit
3 years Well-Child Visit

  • Tests: CBC, Lead
  • Screenings: Hearing, Vision
4 years Well-Child Visit

  • MMR
  • DTaP
  • IPV
  • Varicella
  • Tests: CBC, Lead
  • Screenings: Hearing, Vision
5 - 10 years Well-Child Visit: annually

  • Screenings: Hearing, Vision
11 years Well-Child Visit

  • Tdap booster (tetanus, diphtheria, pertussis)
  • MCV (meningococcal disease)
  • HPV (human papillomavirus) - 3 doses over 6 month period
  • Screenings: Hearing, Vision
12 - 15 years Well-Child Visit

  • Screenings: Hearing, Vision
16 years Well-Child Visit

  • MCV booster
  • Screenings: Hearing, Vision
17 years Well-Child Visit

  • Screenings: Hearing, Vision

 

*Annual Flu shot after 6 months of age
**These vaccines are combined into one injection.
