Health Insurance Assistance and Services at South Boston Community Health Center

Assisting you in accessing the care you need.

The SBCHC Financial Counseling department helps patients to:

  • Submit a Medicaid (MassHealth) application
  • Submit a Medicaid (MassHealth) Renewal form
  • Elect or change an insurance-assigned PCP
  • Select a MassHealth health plan
  • Submit supporting documentation to MassHealth, such as proof of identity, income, or residency
  • Address general Medicare questions

MassHealth Members

Once approved for Medicaid (referred to as "MassHealth" in Massachusetts), you may be eligible to select a MassHealth health plan. Choosing the right plan for you and your family depends on your location and where you receive primary and specialty care.

WellSense Community Alliance (formerly BMC HealthNet Community Alliance) is the MassHealth plan accepted at South Boston Community Health Center (SBCHC).

Most plans require you to have a designated Primary Care Provider (PCP). To ensure your SBCHC visit is covered by insurance, you must select a South Boston Community Health Center provider as your PCP prior to your appointment. To select or change your PCP, you can:

  • Log in to your MassHealth Portal
  • Contact MassHealth Customer Service at 1-800-841-2900, or
  • Book an appointment with our Financial Counseling department at (617) 269-7500 for assistance with updating your PCP assignment.

Uninsured

At SBCHC, we assist individuals in applying for state-sponsored health coverage. With a single state-wide application, you can be screened for eligibility for:

  • MassHealth coverage
  • ConnectorCare Plans
  • Qualified Health Plans

You can apply online on the MA Health Connector Website. If needed, our Certified Application Counselors are available to guide you through the application process. Book an appointment with Financial Counseling by calling (617) 269-7500.

To expedite your application process, please bring the following documents (if applicable):

  • Social Security Card
  • Proof of identity
  • Proof of citizenship
  • Proof of residency

Acceptable Forms of Documentation

Identity
  • U.S. Passport (even if it is expired)
    will satisfied both Identity and Citizenship
  • Driver’s License
  • Legal Permanent Resident card
  • Certificate of U.S. Citizenship
  • Certificate of Naturalization
  • Military ID
  • School ID
  • A government issued ID containing the individual’s photo or other identifying information
  • Military dependent’s ID
  • U.S. Coast Guard Merchant Mariner card
Citizenship
  • U.S. Passport
  • Certificate of Naturalization
  • Certificate of U.S. Citizenship
  • U.S. Birth Certificate
Residency
  • Utility bill or work order dated within the past 60 days
  • Copy of deed and record of most recent mortgage payment
  • Statement from a homeless shelter
  • School records
  • Section 8 agreement
  • Homeowner's insurance agreement
  • Copy of lease and record of most recent rent payment
  • Notarized affidavit supporting residency

Once you've submitted a MassHealth application, you can access South Boston Community Health Center (SBCHC) services while awaiting approval.

For those choosing not to apply for health insurance:

  • Out-of-pocket payments can be made at the time of registration.
  • Patients are required to sign a Financial Responsibility Form to acknowledge responsibility for any additional charges incurred during their visit.

At South Boston Community Health Center, we truly believe in eliminating barriers to care. If for some reason you are not eligible for any of the coverage plans listed on our Accepted Insurance Carriers page, please schedule an appointment with Financial Counseling to discuss other options, including discounts on SBCHC-provided services.

Under-Insured

Patients with private or employee-sponsored insurance plans wishing to apply for a secondary coverage through the state can also book an appointment with Financial Counseling. Our Certified Application Counselors can walk you through an online application for additional coverage that could help with your current copayments.

Medicare

Patients who are approaching 65 years of age, who are preparing to enroll in Medicare, should visit: https://www.medicare.gov/basics/get-started-with-medicare
