How do I sign up for MassHealth?

Visit our Health Insurance Services department to apply for MassHealth. We can assist you with an online or paper application that will screen you for eligibility for all MassHealth coverages, ConnectorCare Plans and Qualified Health Plans. Coming prepared with the necessary documents will expedite your application process.

  • Driver’s License
  • Legal Permanent Resident card
  • Certificate of US citizenship
  • Certificate of naturalization

  • School ID
  • A government issued ID containing the individual’s photo or other identifying information
  • Military dependent’s identity card
  • U.S. Coast Guard Merchant Mariner card

  • Paid weekly: 2 pay stubs
  • Paid bi-weekly: 1 pay stub

  • Self-employed: Federal 1040 Tax return from previous year
  • Statement of military earnings (gross)
  • Letterhead statement of gross weekly earnings (including pay rate,
    number of hours worked, salary, overtime rate if applicable, and
    number of overtime hours)

Health Insurance Services is located on the first floor of our facility at 409 W. Broadway in South Boston and can be reached at 617-269-7500. Hours of Operation are currently Monday through Friday, 9am to 4pm.
