Job Summary
This role is split between direct patient care and care coordination tasks. The Care Coordination Medical Assistant works closely with nurses, providers, and other team members to support patients in both clinical and administrative ways. The ideal candidate is organized, flexible, and comfortable working in a fast-paced outpatient setting with a diverse patient population.
Qualifications
- High School Diploma or Equivalent
- Certification in Medical Assistant Training or enrollment in nursing school, physician assistant school or completion of bachelor’s degree with health science training.
- Minimum 2 years of relevant experience preferred
Representative Job Duties
Clinical Medical Assistant Duties (50%)
- Obtain vital signs, perform point-of-care testing, EKGs, and screenings for primary and specialty care patients.
- Assist nurses and providers with clinical tasks including vaccine and medication administration, procedures, and pre-visit planning.
- Prepare and maintain exam rooms, ensuring cleanliness, proper stocking, and smooth patient flow.
- Greet and room patients, update patient information in the electronic health record (EHR), and assist with scheduling follow-up visits.
- Communicate effectively with patients and providers during visits regarding wait times and care updates.
- Provide excellent customer service and support to patients, ensuring a welcoming and respectful environment.
- Maintain infection control standards and ensure equipment is clean, sterilized, and in working order.
- Transport patients via wheelchair as needed.
Care Coordination Duties (50%)
- Collaborate with nurses and providers to coordinate timely care, including managing health-related paperwork and referrals.
- Facilitate home care referrals, Durable Medical Equipment (DME) requests, and follow-up on external requests.
- Manage incoming and outgoing faxes, forms, and documentation to support patient care continuity.
- Answer phone calls, take messages, and triage non-emergency inquiries appropriately.
- Troubleshoot scheduling issues, delays, or complex patient needs, and assist in reassigning staff or patients as needed.
- Maintain positive working relationships with internal departments and administrative staff to ensure seamless care delivery.
- Track and document all care coordination activities, ensuring timely follow-up and maintaining accurate, detailed records in the electronic health records.
